You probably missed something. Have you created your database in MySQL, using PhpMyAdmin?
A nice way to create it is to simply create a new user (Privileges -> Add a new User), and click on the Create database with same name and grant all privileges.
This way, all the things (creating a user, an access and a specific database) are done in one click.

I use a MysQL 4.0, with an EasyPHP installation.
